By accessing and using the services of Vitae Chiropractic Clinic (hereinafter “the Clinic”), located at 3807 White Horse Rd, Suite C, Greenville, SC 29611, you agree to be legally bound by these terms and conditions. If you do not agree with any part of these terms, please do not use our services.
Vitae Chiropractic Clinic provides chiropractic care services, including but not limited to: spinal adjustments, chiropractic manipulation, rehabilitation therapy, and treatment for motor vehicle accident injuries. All services are performed by licensed and certified professionals.
As a patient, you are responsible for: • Providing accurate and complete medical information • Reporting changes in your health condition • Following treatment recommendations provided • Paying for services in a timely manner • Attending scheduled appointments or canceling with at least 24 hours notice
Although chiropractic care is generally safe, like any medical treatment, there are potential risks. The patient acknowledges being informed about the risks and benefits of chiropractic treatment. Vitae Chiropractic Clinic is not responsible for unexpected results if the patient has not followed treatment recommendations or has withheld important medical information.
Patients must cancel appointments with at least 24 hours notice. Cancellations made with less than 24 hours notice or no-shows may be subject to a cancellation fee. The Clinic reserves the right to charge a no-show fee.
Services must be paid according to the payment policy established by the Clinic. We accept most health insurance. For personal injury cases, we work with personal injury attorneys and can bill directly to the case when appropriate.
The patient acknowledges receiving complete information about diagnosis, treatment plan, risks, and alternatives. The patient has had the opportunity to ask questions and understands the treatment to be received.
Vitae Chiropractic Clinic is not responsible for: (a) treatment results that vary from patient to patient, (b) complications due to undisclosed pre-existing conditions, (c) indirect or consequential damages arising from the use of our services.
Vitae Chiropractic Clinic reserves the right to modify these terms at any time. Changes will be effective immediately and patients will be notified.
These terms are governed by the laws of the State of South Carolina. Any dispute will be resolved in the competent courts of Greenville, South Carolina.
